and Thank You for your interest in one of our Angels.  This is the area of our website where we endeavor to learn as much as possible about you as a potential adopter of one of our residents.  We understand that you might have reservations about furnishing some of the information on this form due to the possibility of identity theft or other fraudulent activity.  We understand & respect your concern & have tried to ask only those questions which we feel are necessary to help us match you with the most compatible pet to fit your home and lifestyle.  We have tailored our form to ask for only that information which is readily available in the public domain from sources such as telephone listings & reverse lookups, county tax records, internet search engines, etc.

In order to help you find your perfect Angel, we ask that you complete this form. Please answer all questions completely and honestly.  If any information is left blank, we will ask you to furnish it.  You may use n/a, none or a "0" where applicable, but please be sure each field contains an entry.  If you are still uncomfortable with the question, please enter WBFL (will be furnished later) & we will discuss that question either personally or by phone.  If you decide to adopt today, this form will become part of your adoption contract. If you do not find your Angel today, this form will remain in our files for 90 days so that you will not have to fill it out again on your next visit.

In order to be considered as an adopter today, you must:

  • Be at least 21 years old
  • Have a valid driver's license or other photo I.D. and identification showing your present address
  • You must have the consent of all the adults living in the household
  • Be able to verify that pets are allowed where you live if renting or otherwise not the property owner
  • You must have proof of pet deposit if required
  • Your current pets must have current vaccinations, be free of contagious illnesses and be spayed or neutered
  • Be able and willing to spend the time and money necessary to provide the proper care for a pet, including but not limited to, food, shelter, veterinary care, grooming, training & recreational toys for the lifetime of the pet you adopt.
  • You must be physically, financially and emotionally able to care for this animal
  • You must agree that any damage done to your home by the pet is NOT the responsibility of Animal Angels

We realize that this form is very long and asks many questions that you might feel are "too personal".  Please put yourself in our position.  Please be truthful.  Don't tell us what you think we want to hear.  Above all, put the welfare of the animals before yourself.  If you are not TOTALLY committed to being a guardian for an animal, please don't ask to adopt it.

Animal Angels is committed to enriching the lives of our animal friends who may be without shelter, food, care or love, regardless of age, natural beauty or condition, and to find them temporary and permanent homes. Our animals are living beings entrusted to our care. We can not guarantee our pets are housebroken.  Housebroken behavior exhibited in the foster home may be different than when in another environment based on schedule, feeding and watering routine.  Should the need arise are you willing to housebreak this pet?

may not be known and you may encounter some behavioral problems, such as, but not limited to barking, digging and chewing?

    It is the policy of
    ANIMAL ANGELS
    to spay or neuter all adopted pets and to provide 
    medical care as needed to bring them to good health & adoptability.  To 
    help cover this expense, a 
    donation of $150.00 would be very much appreciated prior to 
    adoption.  This will help to defray the costs we have incurred while 
    this animal was in our care & to ensure that funds will be available to help 
    the next animal who needs us.   
    

    The donation is tax deductible to the extent allowable 
    under Federal tax laws.
 If 
        the animal is diagnosed with a medical problem within 10 days of 
        adoption, it may be returned to Animal Angels.

        
I agree to
        the following provisions:

        
1. I 
        agree to take the dog to a veterinarian for routine examinations, to 
        keep the animal free of parasites, properly vaccinated, including annual 
        rabies vaccinations, and on heartworm preventative, and to provide 
        immediate veterinary care in the event the animal becomes ill or is 
        injured. I agree the dog will not be subjected to cosmetic surgery, 
        including but not limited to ear cropping and tail docking.

        
2. I 
        agree to provide not only creature comforts, but also to love and 
        provide the emotional security necessary to the animal's well being. I 
        agree to provide the dog with all the care and attention necessary to 
        ensure the health and well being of the dog. This will include providing 
        at all times food, fresh water, exercise and medical care, grooming and 
        a loving safe healthy environment for the dog.

        
3. I 
        agree to have proper collar and an identification tag with current 
        information on my pet at all times, event if he/she is always indoors.
        NOTE: A CHOKE CHAIN 
        SHALL ONLY BE USED WHEN TRAINING OR WALKING (CAN BE USED IN CONJUNCTION 
        WITH A REGULAR COLLAR) BUT MUST BE REMOVED WHEN DOG IS OFF LEASH.

        
4. I 
        agree my pet will be kept indoors (outdoors only with direct 
        supervision) . I will keep this dog as a house pet, as a member of the 
        family. The dog will never be used for purposes other than that of a 
        family companion. I agree to never allow the dog to be used for an 
        experimental purpose, nor will I allow the animal to be chained, life 
        kenneled or used for guard purposes. He/She is not a primarily outside 
        dog. This pet will not ride in the back of a truck or open jeep unless 
        in a crate which is secured.

        
5. I 
        will crate/cage the dog as recommended for purposes of achieving and 
        maintaining housetraining as well as to decrease the likelihood of the 
        dog injuring him/herself or my property when I am unable to supervise or 
        am away from the premises. I will continue to use the technique as long 
        as necessary to prevent elimination and/or destruction problems. The 
        crate will be kept within the living area of my home.

        
6. I 
        agree to comply with all state and local laws pertaining to ownership of 
        the dog, including those laws requiring licensing of the dog within 30 
        days. I agree to notify Animal Angels immediately if the dog becomes 
        lost. I agree to redeem the dog promptly (within 24-36 hours) if the dog 
        is picked up by an animal control agency.

        
7. 
        This adoption agreement is nontransferable and change of ownership voids 
        this contract. If I am unwilling or unable to retain ownership of the 
        animal, I understand that I am to return the animal to Animal Angels. 
        I WILL NOT SELL, 
        GIVE AWAY OR TURN THE DOG OVER TO ANY PERSON(S), AGENCY OR ORGANIZATION 
        (including Animal Control) WITHOUT THE EXPRESS WRITTEN PERMISSION OF 
        ANIMAL ANGELS. ANY TRANSFER OF THE DOG WITHOUT WRITTEN PERMISSION WILL 
        BE MADE KNOWINGLY AND FRAUDULENTLY AND IS A BREACH OF CONTRACT.

        
8. I 
        agree to allow Animal Angels to inspect the home environment and yard, 
        and if any violations of this contract is in evidence, I agree to allow 
        the agent of Animal Angels to remove the animal from the premises 
        occupied by me, and entry shall not constitute a trespass.

        
9. I 
        understand and agree that any breach of one through eight of this 
        contract constitutes grounds for Animal Angels to rescind this contract, 
        in which case ownership will revert to Animal Angels. The animal must be 
        surrendered to an Animal Angel's agent upon demand. I agree to pay the 
        court costs and reasonable attorney fees incurred by Animal Angels in 
        enforcing this contract.

        
10. I 
        agree to pay Animal Angels the sum of $2,000 as liquidated damages in 
        the event the terms of this contract are breached; this liquidated 
        damage value being agreed to for the purpose of establishing value of 
        the animal and does not bar Animal Angels from seeking return of the dog 
        by a judicial process or other legal means.
